Registration Rates

Full Convention Rates :

Early Bird Rate 

Member: 1,500 PhP

Undergraduate: 1,500 PHP

Non-Member: 2,500 PHP

Foreign: 50 USD

Until November 16, 2021

Regular Rate 

Member: 2,500 PhP

Undergraduate: 2,500 PHP

Non-Member: 3,500 PHP

Foreign: 75 USD

November 17-December 3 2021

One-day Convention  Rates:

Early Bird: 1,000 PhP; Regular: 2,000 PhP

Registration Guidelines

Please read through the instructions for registration:

  • Please take note of the registration fees for the Impetus 2021 and payment options via bank deposit.

  • Deposit slip/s to be uploaded in the registration form for individual or group payments should contain the name/s together with the amount paid per person.

  • One registration form shall be completed per individual. For those who paid in groups, each delegate must accomplish the registration form individually.

  • Upon submission of the requirements for online registration, please expect an email with the details submitted by the delegate immediately thereafter as well as another email confirming the receipt of registration within 5 working days.

  • If confirmation was not received, please contact the IMPETUS 2021 Registration Committee through pptaimpetus@gmail.com.

  • REGISTRATION shall not be considered valid WITHOUT receipt of the confirmation email.

  • The registration fee is non-refundable but transferable.

Reminders:​

  1. Online registration, bank deposits, and online payment shall only be accepted until December 3, 2021, 11:59 PM.

  2. The name provided during the registration process shall be used when printing the CERTIFICATE OF ATTENDANCE.

  3. The registration is not tax inclusive.  (PPTA TIN: 205-634-769-000)

  • Account Name: Philippine Physical Therapy Association 

 

STEP 2: Prepare necessary documents for registration

  • Image of updated Identification Card (Professional-PRC ID or Company ID; Undergraduate student: School ID)

  • Image of Proof of Membership to PPTA (ID, Confirmation document or Certification of Membership)

  • Image of Deposit Slip

STEP 3: Click link to registration form below

STEP 4: Fill up and submit registration form

STEP 5: Wait for the confirmation email from the registration committee
