Registration Procedures

Please read through the instructions for registration:

  • Please take note of the registration fees for the Impetus Pre-convention 2019 and payment options via bank deposit and/or PayPal.

  • Deposit slip/s to be uploaded in the registration form for individual or group payments should contain the name/s together with the amount paid per person.

  • One registration form shall be completed per individual. For those who paid in groups, each delegate must accomplish the registration form individually.

  • A participant may attend more than one workshop. He/she will have to submit one registration per workshop that he/she intends to attend.

  • Upon submission of the requirements for online registration, please expect an email with the details submitted by the delegate immediately thereafter as well as another email confirming the receipt of registration within 5 working days.

  • If confirmation was not received, please contact the IMPETUS 2019 Registration Committee through pptaconventions@gmail.com.

  • REGISTRATION shall not be considered valid WITHOUT receipt of the confirmation email.

  • Registration fee is non-refundable but transferable.

  • The name provided during the registration process shall be used when printing the CERTIFICATE OF ATTENDANCE.

Offline Registration Procedures (over-the-counter bank transaction)

STEP 1: Deposit payment to organizer ‘s account

  • Bank and Branch: Security Bank - University of Santo Tomas Branch

  • Account Number: 0000 - 014406 - 441

  • Account Name: Philippine Physical Therapy Association 

(Note: Must be spelled out completely in the deposit slip)

STEP 2: Prepare necessary documents for offline registration

  • Image of updated Identification Card (Professional-PRC ID or Company ID; Undergraduate student: School ID)

  • Image of Proof of Membership to PPTA (ID, Confirmation document or Certification of Membership)

  • Image of Deposit Slip

STEP 3: Click link to registration form below

STEP 4: Fill up and submit registration form

STEP 5: Wait for the confirmation email from the registration committee

Online Registration Procedures (online payment)

STEP 1: Deposit payment to organizer‘s account

Note:

  • Make sure to cover the corresponding service fees of this transaction type. For more details, refer to this link.​​

    • For local or domestic transactions, a service fee of 3.9% + PHP 15.00 will be charged on top of the required amount. Rates and PayPal service fees are as follows:

      • For rate amounting to PHP 800, kindly pay PHP 848.08

      • For rate amounting to PHP 1200, kindly pay PHP 1,264.31

      • For rate amounting to PHP 1600, kindly pay PHP 1,680.55

    • For international transactions, the service fee is 4.4% + PHP 15.00.

  • Do not forget to take screenshot of completed payment
